What is Commando Surgery?
COMMANDO (Combined Mandibulectomy and Neck Dissection Operation) is a surgical procedure performed to remove the primary tumour surrounding the cervical lymph nodes. Generally, the procedure involves treatment of the bottom of the lower jaw. The doctor examines and operates on the lymphatic system of the neck, the sternocleidomastoid muscle, and the internal jugular vein. Command surgeries also require subsequent facial reconstruction to reshape the facial structure that was degenerated during surgery. Surgical reconstruction occurs after the cancer has been removed from the head and neck. The entire procedure is complex and should only be performed by an experienced surgeon.
Cancers in the oropharynx are removed with the help of a command operation. Surgeons resort to this procedure to cure oral cancer that may be present on or around the tongue, floor of the mouth, larynx, pharynx, upper jaw, lower jaw, or roof of the mouth. Cancers that spread to the head and neck follow their path from the head to the neck and chest. Therefore, the surgery involves a compound resection to prevent cancer from spreading. Also, head and neck tumors tend to move to one side of the body.
In order to stop the cancer pathway, a dissection of the lymphatic channels is therefore performed.
